Medical Forms for the 2024-25 School Year

All items for the 2024-25 school year must be completed in Magnus by August 3, 2024.

Please note: Magnus Health is no longer accepting forms that are mailed in. Completed forms can be scanned/photos taken and uploaded to your account. They can also be faxed to Magnus or uploaded through the Magnus app. For more information on using the app, click here.

Returning Students 

1. Important Notes about the Magnus Health requirements: (These items need to be completed by the due date in your account.)

  • Student Health Record--all the information that was entered last year is saved in your account. You will only have to make updates and confirm what is already in your account.
  • Annual Physical and Sports Form*--the physical and sports form that is already in your account is valid for one year from the date that the physical exam was performed. A new physical and sports form must be uploaded by the due date in your individual account. (The form must be signed, dated and stamped by the doctor's office.)
  • Immunization Form*--the complete immunization record that was previously uploaded will be saved in your account. You will only need to upload any new immunizations that are required.

*St. Saviour only accepts our medical forms; we do not accept NYC DOE forms, camp forms, etc.. Copies of SSHS medical forms can be found in your Magnus account.

 2. These items need to be completed again:

  • Consent to Treat--this question will need to be answered again and the form electronically signed.
  • Proof of Insurance--this form needs to be printed, completed, have an original parent signature and uploaded again.

  3. Complete these requirements only if they apply to your daughter:

  • Sports Permission Form if applicable--this form needs to be printed, completed, have an original parent signature and uploaded again.
  • Asthma Action Plan Form, Food Allergy Action Plan Form, Diabetes Action Plan Form and Seizure Action Plan Form- if applicable--this yes or no question will have to be answered again and if applicable the form needs to be printed, completed, have original parent and doctor signatures and uploaded again.

New Students

Here is a checklist of things that should be entered into Magnus: (Copies of all of the forms can be printed from your Magnus account--click the print blank forms button. Once they are completed, please upload them back into your daughter's account.)

  • Student Health Record

Class of 2028 and transfer students--you will receive an email with information on creating your Magnus username and password on Tuesday, June 11, 2024.
